I declare that I meet the membership requirements set forth in Article (3) of the Statute.I commit to the values, objectives, and principles of the Network as stated in the Statute.I undertake not to engage in any political activity under the auspices of the Network.I undertake to pay the membership and annual subscription fees upon preliminary approval, as a condition for final admission in accordance with Article (4).I consent to the processing of my personal data exclusively for Network purposes and in a manner that ensures confidentiality.

I authorize the Executive Office to review and decide on my application in accordance with the prescribed procedures, and I acknowledge my right to respond and present a defense in the event of any action taken under Article (6).
